At James and Rosemary Phalen Leadership Academy, all students are provided a healthy, nutritious breakfast and lunch at no cost through the National School Breakfast and Lunch Program.
If your student has an allergy or needs a food accommodation, please complete a meal accommodation form and return back to the school nurse. All meal accommodation forms must include a physician’s signature.

The letter must contain the complainant’s name, address, telephone number, and a written description of the alleged discriminatory action in sufficient detail to inform the Assistant Secretary for Civil Rights (ASCR) about the nature and date of an alleged civil rights violation. The completed AD-3027 form or letter must be submitted to USDA by:

McKinney-Vento Act

The McKinney-Vento Act protects scholars from discrimination based on their lack of a permanent residence. Homeless Scholars are eligible to receive a variety of services including: transportation services, free meals under the school nutrition programs, limited English proficiency class, gifted and talented programs, and special education services based on a scholar’s eligibility.
